In 2020, Cynthia Nixon gained even more recognition for her role in the hit Netflix drama Ratched, further elevating her career. Outside of acting, Nixon is a strong advocate for LGBTQ+ rights, particularly same-sex marriage, identifying as a lesbian herself.

She met Christine Marinoni, her future wife, at a lesbian rights rally in 2002, and they announced their engagement during the 2009 same-sex marriage march in New York. Nixon's portrayal of Miranda in Sex and the City often hinted at the idea that romantic preferences are fluid, rather than strictly defined.
