My coworker had a patient who had recently undergone an esophageal repair and had an NG tube sutured in place.

Somehow, the tube came out, and without thinking, she tried to put it back in. Unfortunately, that caused the patient to start bleeding, tear the esophagus, and sent them straight back to the OR! Yikes!
